Required PE Advisory

    The district requires that each PE student and a parent complete our PE advisory form when starting the online PE course. Students must complete the form AND parents must complete the form, so you both have to complete the form. If you do not complete the advisory requirements, you will be removed from the online PE course.


    Polar Monitor Check-Out
    Students will be using a Polar heart monitor owned by the school district and issued to the student for the duration of their current PE enrollment. The monitor is checked out to the student in the same manner as a textbook or other school property is assigned to a student. Students are responsible for damage and replacement cost of the Polar monitor. The Ignite monitor replacement cost is $200 and the charger cord is $10. Only the Polar models owned by the district can be used in our online PE course; other brands and models are incompatible with the structure and function of our PE course. Fees for damage and loss are charged to the student's district fee account.

    Physical Activity Advisory
    Online PE requires students to complete workouts that are recorded using a Polar heart rate monitor. Students should ensure that they have an adult or friend who can provide assistance should an urgent medical situation arise during workouts.

    Students Are Advised The Following:

    • If you have a known health condition or physical injury – prior to starting the course, or during the course – that might impact your ability to complete the physical workouts, you must immediately notify your online instructor to determine your ability to continue.  A physician’s evaluation of the injury may be advised, and a physician’s note may be required in order to modify physical activity requirements.
    • While completing workouts, you should remain aware of your physical condition and avoid exertion that leads to physical injury.
    • While completing the workouts, you should maintain your heart rate within zones 2-5 with most of the workout time spent in zone 3. Avoid prolonged time in zone 5. 
    • If at any time you feel chest pain, unusual difficulty breathing, or any other abnormal physical pain while completing workouts, you should immediately stop activity and seek medical attention. 
    • eAcademy always recommends exercising with a buddy. A friend or parent to work out with you or even just observe you, can help in case you need medical assistance.

    Student/Parent/Guardian Acknowledgement: 

    • By signing online, students and parents/guardians acknowledge that they fully understand and accept the equipment responsibilities and risks associated with engaging in physical activity as part of eAcademy Online PE.
    • Students and parents/guardians further agree to adhere to the precautions listed on this form and to use good judgment in deciding when to avoid or discontinue participation if the student shows any signs of having difficulty breathing, chest pains or other significant health issues.

    Remember that completion of the advisory is not optional. Students who fail to complete the advisory will be removed from the online PE course.
