Online Required Athletic Forms

All athletic forms are submitted online through TouchBase for Olathe high schools and the following middle schools: California Trail, Chisholm Trail, Frontier Trail, Indian Trail, Mission Trail, Prairie Trail and Summit Trail.
All other middle schools use printed PDF registration forms (Oregon Trail, Pioneer Trail and Santa Fe Trail).
Five Steps for Online Physical Registration
- Print the pre-participation physical form and take it to the physician's visit.
- Log in to Touch Base, choose your student and then choose "Registrations."
- Choose "Register" next to the activity or athletics registration tab, then choose "Manage."
- Complete and sign the concussion, student transportation and substance abuse forms.
- Upload the completed Pre-Participation Physical as one document and "Submit." You will be notified by email of acceptance. This is required only once, regardless of multi-sport tryouts.

Those who don't have a scanner can use their smart phone for the Pre-Participation Physical form.
- Download a scanner app.
- Snap a picture of each page of the document.
- Save all five pages as one document and upload it from your computer or phone.

Pre-Participation Physical Evaluation
Physical examinations are required for all students Grade 6-12 who are participating in sports. This form was updated in April 2023.

Concussion Pre-Season Release Form
The form must be on file before the student participates in his or her first practice of the school year.

Please review the transportation options and return the signed form to school.
